Near the end of each calendar quarter, L&I will mail you the Employer's Quarterly Report for Industrial Insurance form to use for submitting hours or other reporting units. The form includes instructions and contact information or call 360‑902‑4817.

| Due dates falling on non-business days Per RCW 1.12.070 (3) (www.leg.wa.gov), "If the date for filing any report, claim, tax return, statement, remittance, or other document falls upon a Saturday, Sunday or legal holiday, the filing shall be considered timely if performed on the next business day." |
