Address changes

Any change of address for retrospective rating participants must be in writing (either by postal mail or fax). The request should include:

• Business/organization name.
• L&I account ID number and Retro ID number or UBI (Unified Business Identifier) number.
• Name/telephone of person to contact if there are any questions.
