In order to pay you promptly for providing service to injured workers, you must have an active provider account with L&I.
To apply:
Important note: You must have an individual provider account number, even if you are in a group practice.
Once you have a provider account number please notify us of any changes to your address, or business status.
To change your account information:
For other account changes, call the Provider hotline at 1-800-848-0811. You can also review your listing on our Find a Doctor site.
