Becoming an L&I Provider
 

Apply for your required L&I Provider Account Number

In order to pay you promptly for providing service to injured workers, you must have an active provider account with L&I.

To apply:

  1. Complete a Provider Account Application and Form W-9 (F248-011-000).
  2. Mail your completed forms to the L&I address on the form.

Important note: You must have an individual provider account number, even if you are in a group practice.

Prevent payment delays: Keep us informed of changes to your account

Once you have a provider account number please notify us of any changes to your address, or business status.

To change your account information:

  1. Complete the Provider Accounts Change Form (F245-365-000).
  2. Mail your completed form to the L&I address on the form.
