Independent Medical Examiner (IME)

What you need to know about becoming an independent medical examiner

  • To be paid for independent medical exams for L&I, you must be approved and be assigned an IME provider account number.
  • You must have an individual IME provider account number, even if you are affiliated with an IME firm.
  • You must reapply every 3 years.
  • You must understand L&I requirements for Independent Medical Exams.

To apply as an IME provider

When L&I approves your application, we will notify you in writing.

  1. Complete the Medical Examiner's Handbook Certification Test and print a copy of the certificate of completion to submit with your application packet.
  2. Print and complete the IME Provider Account Application packet (F245-046-000).
  3. Mail a copy of your certificate of completion for the test along with the application packet to:

    Provider Review and Education
    PO Box 44322
    Olympia, WA 98504-4322
