How to become an interpreter

To interpret for injured workers and/or crime victims

  1. Be certified as an interpreter, provide a copy of your credentials.
  2. Get an L&I provider account number. Complete the Provider Account Application and Form W-9.
  3. Complete the Submission of Provider Credentials for Interpretive Services form, includes the Provider Account Application needed to get your L&I provider account number.
  4. Submit completed forms and, at the same time, copies of credentials to:

To update your status or other account information.

To prevent payment delays: Please notify us of any changes to your address, or business status:

  1. Complete the Provider Accounts Change Form.
  2. Mail your completed form to the L&I address on the form.
