Self-insured Employer List

Delimited lists of self-insured employers

The files below are not intended for online viewing. They are provided in delimited format specifically for easier data importing and analysis. They must be saved to your system for use.

The full version of the self-insured employer list (976 KB txt / 5 min) contains the following information on each employer:

Name.
Contact individual.
Address and telephone number.
Uniform Business Identifier (UBI) number.
UBI of employer's third-party administrator (the organization that adjudicates the firm's self-insured workers' compensation claims).
Dates of being self-insured.

The short version of the self-insured employer list (331 KB txt / 2 min) contains the following information on each employer:

Name.
Dates of being self insured.
Name and telephone number of employer's third party administrator (the organization that adjudicates the firm's self-insured workers' compensation claims).

The third-party administrator list (38 KB txt) contains the following information on each administrator:

Uniform Business Identifier (UBI) number.
Name.
Contact individual.
Telephone number and address.
