Enrolling a provider
COHEs identify, recruit and enroll providers in their service area. A provider is required to have an L&I provider account number and an orientation to COHE before they complete the COHE supplemental application. Please submit the completed form to your L&I contract manager.

Dis-enrolling a provider
Those providers requesting removal from COHE should complete the dis-enrollment form. Submit the completed form to your L&I contract manager.

Conducting provider orientation
Orientations can be done in person or through technology as long as it is possible to verify that the provider did watch the presentation. Here are examples of provider orientation slides and script:
