Monitoring on-going performance

L&I monitors and documents COHE performance on a regular basis. Performance includes both adherence to the contract terms and contract performance measures.

Process followed if a COHE is not meeting contract terms
If a COHE is: L&I Contract Manager will take this step:
Not meeting contract terms Issues notice.

Not compliant after 15 calendar days

Issues warning letter.

This will document the items that need to be addressed, the steps and documentation needed by the L&I contract manager, and the number of calendar days that will be given to respond.

Not developing probation plan after the number of calendar days specified in the waring letter

Issues contract termination letter.
 
Process followed if a COHE is not meeting performance measures expectations
If a COHE: L&I Contract Manager will take this step:
Misses benchmark on 3 COHE performance measures (1st time) Discuss issues with the COHE.

Misses benchmark on same 3 COHE performance measures (2nd time)

Issues warning letter.
Misses benchmark on same 3 COHE performance measures (3rd time)

Issues probation letter.

This will document the items that need to be addressed, the steps and documentation needed by the L&I contract manager, and the number of calendar days that will be given to respond.

Does not complete probation plan or is not successful in executing probation plan. Issues termination of services letter.

Annual Review

L&I contract managers are required to perform an Annual Review each June. The goal of this review is to assess and document COHE successes and challenges in the past year and plans for the coming year.

Annual process flow in April-June:

  1. Contract manager review of COHE documentation such as Action Plan, Quarterly Status Reports, and COHE performance measures to date.
  2. Contract manager to request any additional documentation from COHE Director be sent.
  3. Contract manager set up date for on-site COHE visit (typically 30 days before the visit).
  4. Contract manager conducts an on-site visit, during which they:
    • Review documentation on-site.
    • May have brief interviews with:
      • Medical Director.
      • Health Services Coordinators.
      • Attending Providers in the Program of L&I contract manager's choosing.
      • COHE Advisor of L&I contract manager's choosing.
      • Community outreach staff, provider trainer staff, or clerical staff.
      • Employer, patient, or business and/or labor stakeholders
  5. Contract manager writes a summary report on all above and sends it to the COHE director(s). They may, if they wish, send a response which will be incorporated into the final version.

In addition to the Annual Review process, the L&I Contract Manager will send a completed Performance Measurement form to the COHE for comment:
