The COHE Program currently has 4 quality indicators:

• Report of Accident (ROA) to L&I in 2 business days.
• Activity Prescription Form (APF) upon first visit and any time injured worker restrictions change.
• Providers contacting the employer of injury to discuss restrictions and return-to-work plans.
• Completing a Barriers to Recovery Assessment when needed, and no later than after 4 weeks of time-loss.
