Change to the vocational referral process
The Vocational Improvement Initiative was created to improve vocational rehabilitation services by eliminating delays, reducing costs, and improving outcomes for injured workers. As part of the initiative, one strategy for Vocational Provider Education and Accountability limited vocational rehabilitation counselors to 3 contiguous service locations in which they may receive State Fund vocational referrals.
The process for identifying the 3 contiguous service locations was implemented on March 1, 2007. To help providers identify service locations L&I created:
- Vocational Provider Service Locations that lists of all possible 3 contiguous service locations.
- Changes to the Vocational Referral Process for answers to frequently asked questions.
City Names and Associated Service Locations (477 KB PDF / 3 min) that lists all of Washington's cities and towns with their associated service locations.

Becoming a vocational provider in Washington
Read more about vocational provider qualifications then complete and submit one or both of the following forms:
- Vocational Provider Application (F252‑017‑000).
- Intern Supplemental Application (34 KB doc). Interns must also submit this supplemental application.

Submit one of the following forms.
- Firms: Firm Vocational Provider Account Change (F252‑022‑000).
- Individuals: Individual Vocational Provider Account Change (F252‑021‑000).
Some changes will require filing a new vocational provider application. Please refer to instructions on the forms above.
