Earn 1 CEU by taking a quiz on the new Eligible AWA closing report.

This quiz is "open book" and will take approximately 20 minutes. There are 12 questions based on a story about an injured worker named Dan. You can retake the quiz until you are able to answer all questions correctly.

How do I prepare for the quiz?

  • Watch the four training videos entitled "Completing the Eligible AWA Format"(Video page will open in a new window.)
  • Review the new form.
  • For reference during the quiz, consider printing the form, the instructions, and the story about Dan.

  • Have your membership number available for CRCC and/or CDMS.

What should I know while taking the quiz?

Once you begin answering the questions, you should finish the quiz. If there is no activity, the screen may time out after 20 minutes. You will also be unable to return to previous questions without erasing your stored answers.

What should I do after taking the quiz?

  1. Print the certificate immediately after successfully completing the quiz to prevent having to take the quiz again.
  2. Participants are responsible for submitting certificates to the credentialing organization(s) at:
    • Commission on Rehabilitation Counselor Certification (CRCC)
