What information must a provider include in a job analysis?
About Stand-Alone Job Analysis
A Stand-Alone Job Analysis (SAJA) referral is made if there isn't already an open vocational referral. SAJA authorizes a vocational provider to conduct a job analysis when no other services are requested. The provider must:
- Conduct the job analysis on-site whenever possible.
- Comply with WAC 296-19A-170 (www.leg.wa.gov).
The assigned vocational provider:
- May ask the claim manager to authorize assistance from other disciplines (e.g. physical or occupational therapists) in completing the job analysis.
- Is responsible for all work performed on a referral.
- Must complete and submit a SAJA referral to L&I within 15 calendar days from the date of the electronic referral.
