File an Employer Report of Accident (ROA) for an injured employee
The online Employer Report of Accident (ROA) is available in our Claim & Account Center.
File the Report of Accident online
This should take 10 to 20 minutes.
To protect patient information, you will be logged out after 20 minutes of inactivity. You cannot save your work until you have completed all the steps.
Instructions:
- Sign up for secure access (if you don't already have a login).
- Choose "Claim & Account Center" from the list of Labor & Industries services.
- Within the Claim & Account Center; choose "Send information to L&I" then "Complete Employer Report of Accident."

- Tell us who you are.
- Choose your role:
- Injured worker or representative.
- Healthcare or other provider.
- Activate your account.
- Login to My Secure L&I.
- Choose from either service:
- File Worker Report of Accident.
- File Provider Report of Accident.
