  • OR Print, complete, and fax the fillable Video Reservation form (160 K) to 360-902-5675, or mail it to:

  • Read the Video Library Policies
• Print, complete, and fax the fillable Borrower Agreement form (150 K) and fax it to 360-902-5675, or mail it to:

• Include the fillable Video Reservation form (160K) with your first request along with your completed Borrower's agreement.
