Accident Investigation Basics

Course description

The best way to prevent future accidents is to determine the "root" cause(s) of incidents that are currently occurring and then to implement the changes necessary to correct the root cause(s). This workshop will provide participants with an overview of basic accident investigation principles and introduce them to the concept of root cause analysis including: What is the difference between an incident and an accident and why both should be investigated. How to develop a plan for investigating accidents and the steps to take when conducting accident investigations. What should be included in the accident investigation report. An overview of root cause analysis (RCA) and an introduction to some of the more basic RCA tools. The Washington State requirements for accident reporting and investigation. Workshop designed for: Employers who are interested in learning how to develop and implement an effective action plan for conducting accident investigations in their workplace. This is a basic course in accident investigation and not a comprehensive course on root cause analysis.
