Service Alert

If there's a state government shutdown due to lack of a 2017 – 2019 Washington budget, many services provided by the Department of Labor & Industries would not be available starting July 1. L&I has a full list of service interruptions should there be a full or partial shutdown.

Report a death, in-patient hospitalization, amputation or loss of an eye: 1-800-423-7233.

Descriptions of Recent Fatalities

  • A worker was fatally injured after falling approximately 30 feet through a skylight while cleaning a roof. (6/6/17)
  • A worker was seriously injured but later died from those injuries after a heavy metal injection mold, used for manufacturing plastic parts, fell off a shelf onto the worker.(5/30/17)
  • A worker was fatally injured after a truck, the worker was operating, hit a power pole and burned up.(5/15/17)
  • A truck driver was fatally injured after a container, the truck driver was hauling, became dislodged from the chassis and impacted the cab of the truck.(5/11/17)
