Want to participate in the survey, but didn't receive one in the mail?

Here's how to do it:

1. Print the Survey Information Letter (743 KB PDF / 2 min) and read it carefully for instructions.
2. Click on the appropriate trade name (surveys previously completed are listed in the table below as examples) to open and print the appropriate survey form. Fill it out and mail to L&I at the address on the form.
3. Return the survey by the due date, found in the upper right hand corner of the survey form.
