Here are common questions Prevailing Wage Intents and Affidavits (PWIA) users have asked. They are organized in the following categories:

To view approved forms, perform the following steps:
- Click this link, http://www.lni.wa.gov,
or enter this address in your browser Address box and then press
ENTER, or click the Go button.
- From the Labor & Industries home page, click Trades &
Licensing.
- Within the left-side navigation menu, click Prevailing
Wage.
- Under Prevailing Wage, click Intents & Affidavits.
- Under Intents & Affidavits, click View Approved Intents
& Affidavits.
- Click View Approved Prevailing Wage Intents & Affidavits
again on the next page.
- You can search for a form, by entering the form ID number; in
the Form Type drop-down menu, click the selection
for form type (Intent, Affidavit, or both), and then click Search.
- If you do not know the form ID number, proceed to next step.
- From the Value drop-down menu for Form Type, click to select Intents, Affidavits, or Intents and Affidavits.
- In one or more of the Value text boxes, enter your search criteria:
Company Name. etc.; from the Match by drop-down menu, click to select
Exact Match, Begins with or Contains, and then click Search.
- To sort the search results, use the Sort text boxes, enter the
number you wish to sort by from 1-4 in the box on the row of that
search criteria. For example, if you want your search results to
sort first by County (A-Z), enter a 1 in the Sort
text box of the County row.
Getting Started: Creating Intents & Affidavits Forms Online
To create a new Intent form, perform the following steps:
- Log in to the Prevailing Wage Intents and Affidavits (PWIA) system.
- From Intents & Affidavits Main Menu, click Create a New
Intent.
- Under COMPANY, click the Select Company Name button, and then click the line with your Company Name in it to paste
it on to the form.
- Under PROJECT, click Select Public Agency.
Enter the name in the dialogue box. Then click on the correct agency name
to paste it on to the form.
- Complete the remaining fields.
- Dates must be entered in MM/DD/YYYY format.
- Click the Select Prime Contractor Name button. Enter
the name in the dialogue box. Then click on the correct company name to
paste it on to the form.
- Complete the remaining fields, including the Owner/Operator box by entering
the number of owners on the job.
- Under EMPLOYEES' WAGES, click Select Journey Level
Trade of employees who will work on the project.
- Click to select all desired occupations, and then specify the number
of workers needed. Click the Paste Selection button or
press ENTER.
- From the Wage 00.00, Fringe 00.00,
and # of workers text boxes, enter hourly wage, hourly
fringe amount, and the number of workers for each occupation.
- To save a draft copy (recommended), click Save Without
Submitting. Review the form. When the form is complete and the information
is correct, click Submit for Approval.
- In the Card Number text box, enter your credit card number.
- From the Expiration Date drop-down menus, click to select the month
and year of your card expiration date.
- In the text boxes provided, enter your First Name, Last Name, Street/PO Box,
City, State, and Zip Code.
- Click Submit.
What do I do if the public agency I am looking
for is not listed?
Please double check the information you are entering to locate the public
agency and try again. Also, try entering the first word in the public agencies title. For example if you are searching for KING COUNTY EMERGENCY MANAGEMENT DIVISION, first try entering KING. If you are still unable to locate the public agency,
please contact the Prevailing Wage Section by sending an e-mail to

Can I use the system if I don't have a contractor's
registration number?
Yes. You can use the Prevailing Wage Intents and Affidavits (PWIA) system
without a contractor's registration license number if you perform service
type work such as Building Services, Surveying, and Truck Driving. When
you register without a contractor's registration license number, the registration
is automatically submitted as a non-contractor registration. The non-contractor
registration requires manual approval.

How do I create a new Affidavit form?
To create a new Affidavit form, perform the following steps:
- Log in to the Prevailing Wage Intents and Affidavits (PWIA) system.
- From the Intents & Affidavits Main Menu, click Generate New Affidvit from an Intent.
- Find the Intent that you need to generate an affidavit from. The list of approved Intent forms defaults to those filed during the current year. Thus, on June 10, 2009 only forms filed June 10, 2008 through June 10, 2009 will appear.
You may change the date range by using the From Date and Thru Date text boxes at the top of the page.
- Select the Intent by clicking anywhere on the line. When the form has opened this will bring up floating yellow button with two sides,
one saying "Generate Affidavit" (from the Intent)
and the other side saying "Print".
- Select "Generate Affidavit".
- Complete the remaining fields which includes: the Project Completion date, and the Owner/Operator box by entering
the number of owners on the job.
- If your company hired subcontractors, click on "Select Subcontractors". Enter
the name in the dialogue box. Then click on the correct company name to
paste it on to the form. Repeat this process to list all subcontractors.
- If you hired additional employees that are not already listed, click Select Journey Level
Trade to list the employees who performed work on the project.
- Click to select all desired occupations, and then specify the number
of workers needed. Click the Paste Selection button or
press ENTER.
- From the Wage 00.00, Fringe 00.00, # of workers, and # of hours worked text boxes, enter hourly wage, hourly fringe amount, the number of workers
for each occupation, and the hours worked by each.
- Under EMPLOYEES' WAGES, click Select Apprentice Trades of apprentices who performed work on the project.
- Click to select all desired occupations and stages of progression. You can assign multiple apprentices to each selected occupation and stage of progression.
- For Washington State Apprentices, select each apprentice by searching for an apprentice's name or registration number. For out-of-state apprentices list the name, registration number and select the state.
- For each apprentice, under FIRST DAY, LAST DAY, WAGE, and FRINGE text boxes, enter the first day on the project, the last day worked on th project, the hourly wage and hourly fringe benefits.
- To save a draft copy (recommended), click Save Without
Submitting. Review the form. When the form is complete and the information
is correct, click Submit for Approval.
- In the Card Number text box, enter the credit card number.
- From the Expiration Date drop-down menus, click to choose the expiration
date.
- In the text boxes provided, enter your First Name, Last Name, Street/PO Box,
City, State, and Zip Code.
- Click Submit.
How do I generate an affidavit from an intent if I did not file the intent with my current Secure Access Washington (SAW) account?
To access Intents that were created in an separate Secure Access Washington (SAW) account, a PWIA Administrator will need to link your current account to an older account, or an account created by a co-worker. If you filed the intent with a paper form, the affidavit will aslo require a paper form. To link accounts you may contact the Prevailing Wage Section by
sending an e-mail to PW1@Lni.wa.gov,
or call the Prevailing Wage Section at 360-902-5335. Please have the following information available when you call, or in the body of your e-mail.
- Company name.
- Company UBI number.
- Your current Log In ID. (This is the username that you use to access SAW.)
- The Intent ID number that you need to file an Affidavit from.
How do I look up prevailing wage rates when I'm in the Prevailing Wage Intents and Affidavits (PWIA) system?
To look up prevailing wage rates, perform the following steps:
- Log in to the Prevailing Wage Intents and Affidavits (PWIA) system.
- From the Intents & Affidavits Main Menu, click Look-up
prevailing wage rates.
- From the Select an Effective Date drop-down menu, click
to choose an effective date.
- From the Wage Rates by COUNTY drop-down menu, click
to choose the desired county, and then click Journey Wages by
County, or Apprentice Wages by County. A list
of prevailing wages will display.
- The next selection allows you to view all journey wages for a single
trade in all counties. You can also view all apprentice wages for a single
trade in all counties.
If I filed an Intent using the paper form, can I file the corresponding Affidavit online?
No, when you file an Intent using the paper form, you will need to file the Affidavit with a paper form as well.
How do I correct a mistake on a form that I submitted?
To protect the integrity of a form once submitted, the department or customer
cannot change it. A new form must be submitted with any corrections and
another $40 processing fee paid. It is highly recommended that forms be
saved as drafts, without submitting, and reviewed for correctness before
submitting for approval.
Why am I routed back to the User ID and Password
prompt when I try to create a form?
The most common reason is that your bookmark or favorite is no longer usable
to log on to the application. A fresh link may be needed to log on. If you

Managing Your Company Information
How do I change incorrect company information?
Company information is retrieved from the Contract Registration Information
System (CRIS) database. In order to have your contractor registration
or electrical licensing information changed, you will need to contact
Contractor Registration and/or Electrical Licensing directly. Documentation
may need to be submitted to make changes. Company information may include:
address, mailing address, contractor registration number, doing business
as, and business name.
- Contact Contractor Registration at 800-423-7233 for changes to company
information related to contractor registration.
- Send an Electrical Licensing Request for Change of Address form (F500-044-000) for changes to company information
related to electrical licensing information.
How do I change the e-mail address where L&I sends my form approval notification?
To change the e-mail address where L&I sends your form approval notifications
and other notices, do the following:
- Use the Maintain Contact Information section of the Prevailing Wage Intent and Affidavits system client account.
How do I update my PWIA contact information?
You cannot change your address as you have submitted on your
contractor
registration license. You must contact them to change contractor
registration information. To update your contact information, perform
the following steps:
- Log in to the Prevailing Wage Intents and Affidavits (PWIA) system.
- From the Intents & Affidavits Main Menu, click "Maintain Contact
Information."
- From the Intents & Affidavits Contact Information Web page, under
New Information, enter your new contact information. You can change your
e-mail address and your telephone number.
- If you desire, in the optional Registration text box, enter an additional
contractor registration number to use in conjunction with your PWIA registration.
- Click "Save Changes."
Managing Your Online Forms
How do I find a list of forms that have been approved?
To find a list of approved forms, perform the following steps:
- Log in to the Prevailing Wage Intents and Affidavits (PWIA) system.
- From the Intents & Affidavits Main Menu, click Your Approved
Forms.
- A list of approved forms will display. Click the line for the one you
want.
How do I find a list of forms that have been submitted
but are awaiting approval?
To find a list of forms that have been submitted but are awaiting approval,
perform the following steps:
- Log in to the Prevailing Wage Intents and Affidavits (PWIA) system.
- From the Intents & Affidavits Main Menu, click Your Submitted
Forms.
- A list of submitted forms will display. Click the line for the one
you want.
What happened to the forms that no longer appear
in My Approved Forms list?
The list of approved forms defaults to those filed during the current
month. Thus, on June 10, only forms filed June 1 through June 10 will appear.
You may change the date range by using the From Date and
Thru Date text boxes at the top of the page.
How do I find a form that I have saved but not
submitted?
To find a form that has been saved (or returned), perform the following steps:
- Log in to the Prevailing Wage Intents and Affidavits (PWIA) system.
- From the Intents & Affidavits Main Menu, click Your Draft/Returned
Forms.
- A list of draft or returned forms will display. Click the line for the
one you want.
How can I delete drafts of Intents and Affidavits
that I created but do not intend to submit?
To delete drafts of Intents and Affidavits that are created but are not
intended for submission, perform the following steps:
- Log in to your Secure Access Washington (SAW) account and access
your Prevailing Wage Intents and Affidavits (PWIA) system service.
- From the Intents & Affidavits Main Menu, click "Your Draft/Returned
Forms".
- Select the form to be deleted from the Draft/Returned Forms List.
- Scroll down to the bottom of the form and click "Delete Form".
- From the Microsoft Internet Explorer dialog box, click "OK".
How do I find a form that has been returned for corrections?
To find a form that has been returned (or saved as a draft), perform the following steps:
- Log in to the Prevailing Wage Intents and Affidavits (PWIA) system.
- From the Intents & Affidavits Main Menu, click Your Draft/Returned
Forms.
- A list of draft or returned forms will display. Click the line for the
one you want.
How do I resubmit forms that have been returned
for correction?
To resubmit a form, perform the following steps:
- Log in to the Prevailing Wage Intents and Affidavits (PWIA) system.
- From the Intents & Affidavits Main Menu, click Your Draft/Returned
Forms.
- Click on the form you would like to resubmit.
- Complete the requested changes, and then click Submit for Approval.
Help Troubleshooting Problems
How do I report an error?
To report an error, perform the following steps:
- Log in to the Prevailing Wage Intents and Affidavits (PWIA) system.
- From the Intents & Affidavits Main Menu, click Report
an error.
- From the Intents & Affidavits System Error Reporting Web page, in
the Enter the Problem Details Below text box, enter the details of the
error you wish to report.
- Click Send Problem Report.
- If the error is within the PWIA application, please send an e-mail
to PW1@Lni.wa.gov.
When I scroll through the Journey Level Trades,
I cannot paste the selections because the paste button will not activate. Why can't I select the trades?
This issue can occur with telephone modem Internet access users. As stated
in the eligibility requirements, the system may not be reliable for modem
users because of the large amount of data and the data has not finished
loading. Verify that the "Paste Selections" is
visible at the top and the bottom of the list before clicking this button
or the Enter key. The page will become active once it is
fully loaded. Depending on the modem speed, you may still be able to use
it. If the "Paste Selections" button at the bottom
is not visible, the data has not finished loading. It is recommended that
you switch to DSL or other broadband Internet access.
