File a Complaint

If you see a safety or health hazard at your workplace, you may file a complaint with L&I.  Here are instructions and a form for completing a workplace hazard complaint:

Report Safety or Health Hazards in English (127 KB DOC);

If you were demoted, fired or otherwise affected due to raising concerns about safety or health in your workplace, you may file a discrimination complaint with L&I.  Here’s instructions and a form for completing a discrimination complaint:

What L&I will do

  • Respond to a written complaint from an employee or employee representative according to WAC 296-900-12015.
  • Investigate and respond to complaints of workplace safety-and-health-related discrimination according to Chapter 296-360 WAC.
