File your workers' compensation claim online

Step 1: Report it and seek medical treatment

  • Inform your employer of your injury.
  • If you work for a self-insured employer, file your claim directly with them.
  • Seek medical treatment.

Step 2: Gather information

The more information you provide, the faster we can process your claim.

  • Location where the injury occurred.
  • Contact information for any witnesses to the injury.
  • Employer information.
  • Wage information.
  • Names and birthdates of your dependents.
  • If you have already seen a doctor:
    • Your doctor's first and last name.
    • Hospital or clinic where you received treatment.

Step 3: File your claim

  • File the ROA online in My L&I Secure area    (My L&I – Powered by SecureAccess Washington)
  • Or file by phone at 1-877-561-FILE (3453), Monday – Friday, 8 a.m. – 5 p.m.
