Activity Prescription Form (APF)

The Activity Prescription Form (APF) transaction allows providers to communicate a worker’s physical restrictions, ability to work, and treatment plan. Timely and complete APFs ensure that the worker, employer, claim manager, and provider have a shared understanding of the worker's progress.

Claims Information

This transaction allows providers to retrieve a patient's claim number, claim status, accepted diagnoses codes, and accepted diagnoses descriptions. This transaction has the flexibility to be modified to include additional case related information. This is a quick and easy way to get information.
